  • We're looking for an exceptional NDIS Support Coordinator/ Psychosocial Recovery Coach to join our passionate, vibrant and supportive team.

    As a NDIS Support Coordinator, you will work with people with a psychosocial diagnosis to develop and implement their NDIS plans. You will help them to understand their funding, find and connect with providers, and coordinate supports.

    You will also work to build their capacity to manage their own NDIS plans and supports. This is a challenging but rewarding role, and you will make a real difference in the lives of the people you support.

    Front and Centre Support Services is a small boutique provider that supports people with psychosocial disabilities in the Geelong region and surrounds. We provide psychosocial recovery coaching and support coordination services to increase independence, improve quality of life, and connect people with the community.

    Responsibilities

    *Work with people to develop and implement their NDIS plans

    *Help them to understand their funding, find and connect with providers, and coordinate their supports

    *Build their capacity to manage their own NDIS plans and supports

    *Develop and implement recovery plans

    *Meet set KPI’s

    *Keep up-to-date with the latest NDIS policies and procedures

    Qualifications and Experience

    *Tertiary qualifications in Community Services, Mental Health, Psychology, Nursing or similar

    *Experience working as a support co-ordinator, case manager or similar preferred

    *Strong understanding of the NDIS, recovery-oriented practice and trauma informed care

    *NDIS worker screening check, working with children’s check, police check

    *Drivers licence and full comprehensive insurance

    *Excellent communication and interpersonal skills

    *Ability to work independently and as part of a team
